Test Score Requests

To request a copy of test scores be sent from Gainesville State College, print this form, complete it, and fax to (678) 717-3580 or mail to:

ALL parts of the form must be completed before test scores can be sent.  Incomplete requests will be returned.  Scores are mailed out 1-2 business days after receipt of the request.
