Summary of Significant Benefit Changes for 2007

403(b) Plan The general limit increases to $15,000 for the year 2007. Participants 50 years of age or older may contribute an additional $5,000 for the year 2006. You can take advantage of this additional catch-up opportunity provided that you turn 50 years of age at any time during 2006.
457(b) Plan The general limit increases to $15,000 for the year 2007.  Participants 50 years of age or older may contribute an additional $5,000 for the year 2005. You can take advantage of this additional catch-up opportunity provided that you turn 50 years of age at any time during 2006.  As an alternative to the age based “catch up”, participants within three years of normal retirement age may contribute the lesser of twice the applicable dollar limit or the underutilized contribution limit for 2002, 2003, 2004 and/or 2005.
BOR DENTAL No change in coverage or premium.
BLUECHOICE Bariatric surgery will no longer be covered.
9.75 % net premium increase.
KAISER The Kaiser Permanente “Standard Plan” option has been discontinued for Plan Year 2007.  A new $2 million maximum medical and pharmacy lifetime benefit limit has been implemented. The current maximum annual out-of-pocket member costs of $2,000 per individual and $6,000 per family have been eliminated. Home nursing care and home hyperalimentation, will be covered at 100% with a limitation of 120 visits per person per plan year. For inpatient mental health service and substance abuse treatment, the plan will cover 100% for inpatient services after a $200 co-payment per admission; each limited to 30 days per person per plan year. For outpatient substance abuse treatment, the plan will cover 100% for outpatient services after a $25 co-payment per personal visit and/or after a $12 co-payment for a group therapy session.  Personal visits and group therapy sessions will be increased to a combined maximum of 40 visits per person per plan year. Services for care in an extended care facility will be covered at 100% for up to 60 days per calendar year.  . For outpatient mental health services, the plan will cover 100% for outpatient services after a $25 co-payment per personal visit and/or after a $12 co-payment for a group therapy session. Personal visits and group therapy sessions will be limited to a combined maximum of 20 visits per person per plan year.  9.83% net premium increase.
BOR HEALTH New voluntary Obesity Disease State Management Program; pre-certification is required by Unicare.

   2% net premium increase.

 

PPO HEALTH New voluntary Obesity Disease State Management Program; pre-certification is required by Unicare.

   8% net premium increase.

 

Rx Plan New Express Scripts Progressive Drug Management Program and national preferred drug list.  No changes in the co-payment schedule for generic, preferred and non preferred brands.   The Express Scripts national list of preferred drug brands is subject to change throughout the plan year.
BOR LIFE No change in coverage or premium.
DEPENDENT LIFE No change in coverage or premium.
FSAs Regarding Medical and/or Dependent Care Flexible Spending all eligible charges must be incurred no later than December 31, 2005.  All claims documentation for 2005 must be submitted and all outstanding claims issues resolved prior to March 31, 2006. Anyone wishing to participate in this program for the plan year beginning January 1, 2007, must enter a new election on self service during open enrollment.
UNUM LTD No change in coverage or premium.
